This is an excellent opportunity for an experienced Home Health LVN who enjoys working independently while being supported by a collaborative and patient-focused clinical team.

• Provide skilled nursing visits in patients' homes according to physician orders and care plans
• Monitor patient conditions and report changes to the supervising RN and care team
• Educate patients and caregivers on medications, treatments, and disease management
• Perform clinical procedures within LVN scope of practice
• Maintain accurate and timely documentation of patient care
• Collaborate with interdisciplinary team members to ensure quality patient outcomes
• Support patients in achieving their health goals while promoting independence and safety

• Active Texas LVN License
• Minimum one year of clinical nursing experience
• Minimum one year of Home Health experience required
• Current CPR Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong communication, organizational, and documentation skills
• Ability to work independently and manage a daily patient schedule
