Compensation: Estimated Annual Compensation: $70,000 – $86,000. This position is paid on a per-visit basis. The compensation reflected is an estimate of annual earnings for full-time status.

Per-Visit Pay Structure:

• Single visit type structure
• Minimum productivity expectation: 32 visits per week
• Example annual earnings: 32 visits per week × $36 per visit × 52 weeks = $59,904
• Ability to increase visit volume and earnings beyond the minimum requirement
• No cap on earning potential

Austin, TX is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to support patients in a home health setting. This role is ideal for nursing professionals who enjoy working independently while delivering personalized, high-quality care directly in patients’ homes. The LVN will collaborate closely with an interdisciplinary healthcare team to promote positive patient outcomes and continuity of care.

• Conduct skilled nursing visits in patients’ homes in accordance with physician orders and individualized care plans
• Monitor, assess, and evaluate patient conditions, including chronic disease management and post-acute recovery
• Accurately document assessments, interventions, and outcomes in the electronic medical record
• Communicate and collaborate with interdisciplinary healthcare team members to coordinate patient care
• Educate patients and families on medications, care plans, and lifestyle modifications
• Travel to patient homes within the assigned coverage area while maintaining effective time management
• Participate in on-call rotation as required

• Graduate of an accredited practical or vocational nursing program
• Minimum of one (1) year of clinical nursing experience
• Home health experience preferred, but not required

• Active Licensed Vocational Nurse (LVN) license in the State of Texas
• Current CPR or BLS certification
• Valid driver’s license
• Reliable transportation and current auto insurance

• Per-visit pay model based on visit type and complexity
• Typical schedule:
  
  Monday through Friday, 8:00 AM – 5:00 PM, with some flexibility
