Are you an experienced Home Health RN looking for more autonomy, meaningful patient connections, and the opportunity to make a lasting impact in your community?
We're seeking a compassionate and skilled Registered Nurse to join a growing Home Health team serving patients throughout the Austin area. This role offers the opportunity to provide one‑on‑one care in patients' homes while helping them maintain independence, improve outcomes, and age safely in place.
What You'll Do- Perform comprehensive patient assessments and develop individualized care plans
- Complete OASIS documentation and maintain accurate clinical records
- Provide skilled nursing care in the home setting
- Educate patients and families on medications, disease management, and treatment plans
- Collaborate with physicians, therapists, caregivers, and interdisciplinary teams
- Monitor patient progress and coordinate ongoing care needs
- Active Texas RN License
- Minimum 1 year of RN experience
- Home Health experience required
- OASIS experience required
- Current CPR Certification
- Valid driver's license, reliable transportation, and auto insurance
