Home Health LVN / LPN

  • Schedule: Shifts Open Monday - Sunday | 8:00 AM – 5:00 PM

Position Overview

An established home health organization is seeking an experienced and compassionate Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide skilled nursing services to patients in their homes throughout the Austin area. This opportunity is ideal for clinicians who enjoy working independently while building meaningful relationships with patients and their families.

The Home Health LVN/LPN will be responsible for delivering quality nursing care, monitoring patient progress, collaborating with interdisciplinary team members, and ensuring all clinical documentation is completed accurately and in a timely manner.

Primary Responsibilities
  • Conduct skilled nursing visits in accordance with physician orders and individualized plans of care.
  • Observe, assess, and document patients' conditions, reporting significant findings or changes as appropriate.
  • Administer prescribed treatments and interventions while adhering to established clinical standards.
  • Educate patients and caregivers regarding diagnoses, medications, treatments, and ongoing care needs.
  • Maintain thorough, accurate, and compliant medical records and visit documentation.
  • Coordinate care with physicians, therapists, case managers, and other members of the healthcare team.
  • Support quality patient outcomes while promoting safety, dignity, and independence.
  • Participate in an on-call rotation to address patient needs outside of regular business hours.
Qualifications

Required Experience

  • Minimum of six (6) months of recent home health experience required.
  • At least one year of hands‑on clinical nursing experience.
  • Demonstrated ability to work autonomously in the field while effectively prioritizing patient needs.
  • Strong organizational, communication, and documentation skills.

Education & Licensure Requirements

  • Graduate of an accredited Practical Nursing or Vocational Nursing program.
  • Active and unrestricted Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of Texas.
  • Current CPR certification required and maintained throughout employment.
  • Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service territory.
Compensation
  • This position utilizes a productivity‑based compensation structure, with earnings tied to visit volume and completed patient care activities.
  • Eligible candidates will receive a $10,000 sign‑on bonus.
