Private Duty Nurse LVN​/Nights

Join a patient-centered healthcare team as a Licensed Practical/Vocational Nurse (LPN/LVN). In this role, you will provide and document skilled nursing care under RN supervision, following individualized care plans and physician orders. Responsibilities include assessing patient conditions, implementing nursing actions, updating care plans, and educating patients and families on disease management and self-care techniques. You must ensure compliance with safety, infection control, and confidentiality standards while maintaining professional accountability.

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification and TB test within last 12 months
• Six months of hands-on nursing experience preferred
• Reliable transportation

• Attention to detail, time management, effective communication, problem-solving, and organization
• Ability to respond to emergency situations and handle physical demands such as lifting and repositioning patients

• Deliver high-quality, compliant patient care
• Monitor and report patient condition changes
• Maintain accurate documentation and follow policies and regulations
• Educate patients and families on health management
