Job Title: Admission Nurse M-F 1p to 9:30p

Avir at Live Oak is seeking a dedicated Admission Nurse to join our team! As an Admission Nurse, you will play a vital role in ensuring a smooth transition for our patients as they begin their journey with us. Responsibilities include conducting thorough patient assessments, educating patients and families on our services, and communicating effectively with interdisciplinary teams to coordinate patient care.

• Conduct comprehensive patient assessments to identify individual needs and develop personalized care plans
• Provide education and support to patients and families on our services, policies, and procedures
• Coordinate patient care with interdisciplinary teams, including physicians, therapists, and other healthcare professionals
• Maintain accurate and timely records of patient information and assessments
• Collaborate with the clinical team to ensure seamless transitions and quality patient care
• Participate in quality improvement initiatives to drive continuous improvement

• Minimum Years of

• Current RN/LVN licensure in the state of Texas
• Basic Life Support (BLS) certification
• Ability to work effectively in a fast-paced environment with multiple priorities
