We are a family medicine led concierge practice built on private health stewardship – partnering with individuals and multi-generational families to guide health over time.

We provide comprehensive primary care for adults and children, emphasizing continuity, proactive planning, and thoughtful coordination across every stage of life. Our model prioritizes depth, trust, and long-term outcomes, supported by a dedicated care pod designed to protect both the member experience and physician well-being.

The Registered Nurse serves as the clinical backbone of the care pod. You will translate physician direction into action, ensure follow-through across diagnostics and care plans, and act as a consistent point of contact for members. This role comes with real autonomy: you'll build direct, lasting relationships with a small panel of members and largely own how you show up for them – in their homes, at their pace, on your clinical judgment.

You are not only delivering care – you are stewarding it, and you'll see the impact of your coaching and nursing skill unfold over months and years, not just a single visit.

• Travel locally to meet members in their homes, offices, and other appropriate community settings.
• Be comfortable entering members’ homes and work environments as a trusted clinical professional.
• Communicate results clearly and efficiently, escalating when appropriate.
• Coordinate with specialists and external providers to maintain continuity of care.
• Perform assessments, phlebotomy (adult and pediatric), and point-of-care testing.
• Monitor and document clinical data accurately in the EHR, anticipating the needs of members and closing loops without being prompted.
• Reinforce preventive strategies and long-term health plans, and build the kind of trust that lets you coach members toward real, lasting change.
• Advocate for members within the healthcare system when coordination or access is required.
• Participate in developing and refining clinical workflows and policies.
• Protect confidentiality and uphold the highest professional standards.

This role is for a nurse who:

• Associate’s or Bachelor’s degree in Nursing.
• Minimum 4 years of clinical experience (inpatient or outpatient).
• Experience in care coordination or concierge-style settings preferred.
• Full-time (40 hours/week), with meaningful flexibility in how your week is structured – this isn’t a shift-based clinic role.
• High autonomy in managing your schedule and member relationships, with trust placed in your clinical judgment.
