Job Summary

The LCDC Case Worker ensures the provision of recovery-focused, person-centered care with a trauma-informed perspective. This role performs case management, assessment, and service coordination in a dynamic environment, following program guidelines. Key responsibilities include providing continuity of care services, linking consumers and families to needed resources, coordinating, monitoring, and updating services and consumer goals, providing skills training, assisting in crisis prevention, and engaging in community outreach.

The position utilizes assessment, multidisciplinary coordination, and efficient, clinically appropriate allocation of agency resources. A strong working knowledge of substance use disorders, methadone/buprenorphine treatment, and infectious diseases such as hepatitis, HIV, and STD is required. This role may carry an individual caseload and on-call duties may be required. Regular, predictable attendance is essential, and a flexible work schedule may be required.

License and Certifications
  • Current and valid driver’s license with a clean driving record.
  • LCDC or LCDC-CI license.
Experience
  • Experience facilitating group sessions.
Hours and Schedule
  • Full‑time position.
  • Workdays:
    Monday‑Friday.
  • Workhours: 8:00 AM – 9:00 PM (shifts vary).
Salary

$24.18 per hour. The position may be eligible for additional shift differential pay.

  • Second Shift: $3.25 per hour.
  • Third Shift: $7.25 per hour.
  • Weekend Shift: $8.00 per hour.
