Job Summary: The Habilitation Coordinator works alongside a team of advocates to help adults with Intellectual and Developmental Disabilities (IDD) seeking admission to a Medicaid-certified nursing facility who may also have a co-occurring mental health diagnosis, ensuring access to and continuity of care. The day‑to‑day activities follow the federally mandated Preadmission Screening and Resident Review (PASRR) program guidelines, screen people for appropriate placement, use state electronic systems such as CARE and TMHP, and facilitate transition duties when an individual requests to move out of the nursing facility.
The focus may shift to primarily support functions, including assessment, service planning/coordination, monitoring, and crisis prevention/management. The role involves working within a multidisciplinary team and collaborating with community partners to provide a person‑centered approach to care.
Responsibilities:
• Screen and coordinate admission for adults with IDD and co‑occurring conditions under PASRR guidelines.
• Use state electronic systems (CARE and TMHP) to manage records.
• Facilitate transition duties for patients relocating from nursing facilities.
• Perform assessment, service planning/coordination, monitoring, and crisis prevention/management.
• Collaborate with interdisciplinary teams and community partners.
• Maintain a trauma‑sensitive workplace and provide Suicide Safe care.
Education:
- Graduated from an accredited college or university with a Bachelor's degree in Human/Behavioral/Social Services or related field. Master's degree preferred.
License:
- Current and valid driver's license with a clean driving record.
Experience:
- At least 2 years of experience providing services to individuals diagnosed with intellectual and/or developmental disabilities.
Preferred:
- Bilingual in Spanish.
Hours: Relief
Workdays: Monday - Friday
Workhours: 8:00AM - 5:00PM
Salary: $24.18 – $25.39 an hour based on education
