Under the supervision of the Director of Specialized Health and Social Services, and with oversight by the Regional Team Leads/Supervisors, this position provides advanced and comprehensive case management consultation for families with children who have health risks or special health care needs. The employee attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
Oversees case management services for families in the Children with Special Health Care Needs Services Program, CSHCN Program/Waiting list and non‑Medicaid eligible families, pregnant women on Title V or unfunded, children on the CHIP and CHIP Perinatal program, children referred by the Newborn Screening program, children referred by the Childhood Lead Poison Prevention program, children with a lapse in Managed Care Organization or Medicaid coverage, children carved out of the MCO service delivery model, children who are on traditional Medicaid, STAR Health clients remaining in Personal Care Services Program (PCS), and children referred from the MCO’s who need school advocacy, clients on the Personal Care Services (PCS) program, Department of Family and Protective Services (DFPS) and Case Management for Children and Pregnant Women (CPW) programs.
Assists families with eligibility determination for state services and conducts comprehensive assessments to determine the need for PCS/PDN, CSHCN, and CPW services. Travels to conduct home visits, completes intakes, family needs assessments, service plans, and follow‑up. Links families to community resources, communicates program services positively, works as a team member, participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, interagency groups, and community organizations, provides information and referral services, completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner.
Employee participates and/or serves in a supporting role to meet the agency’s obligation for disaster response and/or recovery or Continuity of Operations (COOP) activation; such participation may require an alternate shift pattern assignment and/or location. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Maintains confidentiality.
