Position: LTSS Service Coordinator (Llano and Mason County)

LTSS Service Coordinator (Llano and Mason County)

Schedule:

Monday‑Friday 8am‑5pm CST.

Location:

Primarily a field based position. Candidates must reside in Llano or Mason County. The role allows occasional office attendance for meetings or training. Hybrid or virtual work is not considered unless permitted by law.

Responsibilities
  • Conduct face‑to‑face program assessments using pre‑defined tools.
  • Apply motivational interviewing techniques for evaluations.
  • Coordinate and manage an individual’s waiver (LTSS/IDD, behavioral health, or physical health) and act as the single point of contact.
  • Identify members with potential clinical health‑care needs and coordinate care with clinical teams.
  • Manage non‑clinical needs of members with chronic illnesses, comorbidities, or disabilities.
  • Document short and long‑term service and support goals in collaboration with members and their care team.
  • Identify members who may benefit from an alternative level of service or other waiver programs.
  • Mentor new staff and assist in formal training and process‑improvement initiatives.
  • Submit utilization/authorization requests with supporting documentation.
  • Report critical incidents to state and county agencies and assist with appeals, grievances, and audits.
Minimum Requirements
  • BA/BS degree and a minimum of 2 years of experience with a social work agency, or equivalent education and experience.
Preferred Skills and Experience
  • BA/BS in a health‑care‑related field.
  • Bilingual in English/Spanish (strong preference).
  • Experience in LTSS, case management, social work, or hospital discharge planning.
Job Level

Non‑Management, Non‑Exempt.
