- Plan, organize, develop, and direct social service programs to ensure medical‑related emotional and social needs of residents are met on an individual basis.
- Assess and identify medical‑related social and emotional needs of residents; develop and implement social care plans with the resident, family, physician, and nursing staff.
- Consult with physicians and healthcare team members regarding psychosocial factors relating to resident illnesses, treatments, and discharge plans.
- Provide discharge planning, including referrals to community and family social services.
- Assist residents with financial and legal matters; provide or arrange counseling; help residents make decisions about their health.
- Directly supervise staff; carry out supervisory responsibilities per policies and laws: interview, hire, train, plan, assign, direct work, review performance, reward and discipline employees, address complaints and resolve problems.
- Possess knowledge of federal and state regulations, including Medicare/Medicaid, to arrange needed services not covered by state regulations.
- Assist with resident admission process.
- Orient and train employees on social services policies and procedures.
- Monitor supply and equipment needs and budget.
- Implement policies and procedures to ensure compliance with federal and state regulations.
- Develop and maintain community liaison activities.
- Bachelor’s degree (B.A. or B.S.) in Social Work from a four‑year college or university.
- Minimum of two years of related supervisory experience and/or training.
- ACSW registration.
- Ability to apply existing guidelines, make decisions, and solve moderately complex problems using sound judgment.
- Extensive knowledge of a distinct skill or function and thorough understanding of the organization and work environment.
- Standing, walking, sitting, using hands and fingers, reaching with hands and arms, stooping, kneeling, crouching, crawling.
- Talking or hearing.
- Ability to lift up to 25 pounds.
- Vision required.
- Interaction with co‑workers, residents, or vendors.
- On‑call as needed.
- Possible exposure to communicable diseases and infections.
- Occasional travel.
