Social Services Director

Brookdale Westlake Hills in Austin, TX seeks a Licensed Social Services Director to join our premier CCRC Campus.

Plans, organizes, develops, and directs social service programs to meet residents' medical-related emotional and social needs on a personalized basis.

• Assess and identify residents' medical-related social and emotional needs; develop and implement social care plans with the resident, family, physicians, and nursing staff.
• Consult with physicians and health care team members regarding psychosocial factors affecting illnesses, treatments, and discharge plans.
• Provide discharge planning and referrals to community and family social services.
• Assist residents with financial and legal matters; provide or arrange counseling and help with health decisions.
• Directly supervise staff; perform interviewing, hiring, training, performance reviews, rewards, discipline, and issue resolution.
• Apply federal and state regulations, including Medicare/Medicaid, for required services not covered by state regulations.
• Assist with resident admission and orientation; train staff on social services policies and procedures.
• Monitor supplies, equipment, and budget; implement compliance policies and procedures.
• Develop and maintain community liaison activities.

• Bachelor's degree in Social Work from a four-year college or university.
• Minimum two years of supervisory experience or training.
• ACSW registration required.
• Strong communication and judgment skills; ability to solve moderately complex problems.

Base pay determined by applicant's skills and experience. Temporary associates are not benefits eligible but may participate in the company’s 401(k) program.
