Position Description

Foundation Communities (FC) is seeking a Financial Case Manager to join the Financial Wellness Program. In this collaborative and dynamic role, the Financial Case Manager will support FC residents at-risk for eviction by (1) providing case management for residents experiencing a financial crisis resulting in difficulty meeting rental and utility obligations, and (2) processing monthly applications for emergency rental and utility assistance.

This role will be primarily based at one of FC's Prosper Centers and is expected to travel to FC properties across Austin several days per week to meet with residents.

Primary Responsibilities
  • Maintain a caseload of 12-15 resident households by identifying residents who would benefit from ongoing case management from the monthly pipeline of emergency rental assistance applications and following up on referrals from property staff.
  • Conduct intake assessments, identify client goals, develop service plans, connect clients to internal and external resources, provide case advocacy, monitor ongoing progress, and ensure appropriate case closure or transition to additional services.
  • Utilize motivational interviewing and other trauma-informed, solution-focused strategies to strength client engagement and support goal attainment.
  • Help eligible clients obtain public benefits through Texas Health & Human Services (i.e. Medicaid/CHIP, SNAP, WIC, TANF) by completing applications directly with clients or referring to the Benefits Team when appropriate.
  • Recognize client safety concerns and connect clients to appropriate community resources and crisis services.

Emergency Rental & Utility Assistance Management

  • Take and review applications for emergency rental and utility assistance for 40-50 resident households per month, applying eligibility criteria and knowledge of eviction risk to triage need and approve or deny applications.
  • Communicate application decisions to residents and property staff and submit approved applications through FC’s accounting department or Austin Energy’s Plus 1 program.
  • Collaborate with cross-department staff to evaluate application workflows and improve operational efficiency and accessibility for residents.

General

  • Honor cultural and linguistic diversity of FC residents and aim to provide services in their preferred language directly or through telephone language interpretation.
  • Ensure compliance with program and property requirements, working collaboratively with property management to address resident concerns.
  • Participate in team meetings across departments, including Prosper Programs, Children’s HOME Initiative, and Family Property Management, to build relationships and stay informed about programs and resources.
  • Maintain timely and accurate documentation of case management meetings, virtual client communications (phone calls, emails, and texts), and emergency assistance applications in accordance with HIPAA guidelines and data security standards.
  • Stay appraised of available community resources and best practices in eviction prevention.

  • Proven ability to work independently and as part of a multidisciplinary team
  • Bachelor’s Degree from an accredited college or university in social work, counseling, psychology, or related field with substantial knowledge of poverty, homelessness and mental health issues preferred, and may be substituted with relevant experience
  • A minimum of two (2) years of experience working in a professional setting providing trauma-responsive services to ethnically and socioeconomically diverse and/or unhoused populations required
  • Fluency in Spanish or other language (Arabic, Pashto) highly preferred
  • Access to reliable transportation required
