Case Manager

• Conduct client needs assessments and develop individualized Care Plans
• Connect clients with community resources and government assistance programs
• Provide ongoing encouragement, coaching, and care coordination
• Conduct follow-up calls, home and hospital visits
• Maintain accurate client documentation using Salesforce
• Lead and support client groups
• Collaborate with clinic staff to ensure excellent client care

• Committed Christian with a personal relationship with Jesus Christ
• Passion for serving women, children, and families while upholding the sanctity of human life
• Bachelor's degree in a related social services field, Registered Nurse (RN), or Certified Community Health Worker
• Strong organizational, communication, and computer skills
• Ability to manage confidential information with professionalism
• Case management or care coordination experience preferred
• Experience in coaching, mentoring, or motivational interviewing preferred
• Master's Degree in Social Work (Texas Licensed) preferred

Demonstrates expertise in client needs assessments, care coordination, and case management while maintaining confidentiality and professionalism. Strong ability to connect clients with community resources and provide ongoing support through effective communication and organizational skills.
