Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care across various healthcare settings. The ideal candidate will possess strong clinical skills. This role involves performing patient assessments, administering medications and injections, and supporting patient safety through vital signs monitoring and physical examinations. The LPN will work closely with interdisciplinary teams to ensure comprehensive care delivery in compliance with HIPAA and NCQA standards.

Duties
  • Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations.
  • Administer medications, injections etc.
  • Document all patient interactions accurately
  • Assist with diagnostic evaluations including laboratory specimen collection/processing and medical imaging procedures.
  • Provide specialized care for patients with complex needs such as Alzheimer’s disease, dementia, traumatic injuries (Level I & II Trauma Center experience), infectious diseases, or post-acute recovery.
  • Support discharge planning and case management activities to ensure smooth transitions of care across inpatient and outpatient settings.
  • Educate patients on health coaching topics including nutrition, medication management, family planning, and chronic disease management.
  • Maintain sterile processing standards for procedures such as suturing or spinal taps while adhering to infection control protocols.
Qualifications
  • Valid Licensed Practical Nurse (LPN) license in good standing.
  • Strong understanding of medical terminology and anatomy/physiology relevant to diverse patient populations including individuals with disabilities or developmental delays.
  • Ability to work independently while collaborating effectively within multidisciplinary teams.
  • Excellent documentation skills ensuring compliance with HIPAA regulations and NCQA standards.
