LPN Charge Nurse

Apple Rehab Avon, located in Avon, CT, is a 60‑bed family‑oriented skilled nursing and short‑term rehabilitation facility.

Starting Pay:
Competitive rates up to $37 per hour, based on experience.

• Shift Differentials for 2nd and 3rd shifts and weekends
• Additional Shift Differential:
  Extra $5.00/hour for shifts worked outside scheduled hours

• 24‑hour 3‑11 PT position

• Plan, execute, and oversee nursing services for assigned residents, adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions.
• Efficiently plan and provide professional care aligned with residents’ needs and the changing medical landscape.
• Maintain professionalism in interactions with residents, physicians, families, peers, and support staff.
• Document crucial information related to resident care accurately and comprehensively.
• Administer prescribed medications and ensure accurate record‑keeping in line with facility policies.

• Valid state nursing license in good standing (or in the process of becoming a registered nurse).
• In‑depth knowledge of nursing practices, medical procedures, and long‑term care regulations.
• Strong attention to detail, excellent organizational skills, and ability to manage multiple tasks effectively.
• Collaborative team player with previous experience in long‑term care preferred.

• All LPN positions require weekend on‑call availability to support staff and residents.
• Per diem staff are expected to fulfill two weekend shifts monthly.
