Registered Nurse (Seasoned Nurse 15 plus years experience required)

Our company is a fast growing Independent Care Management and Patient Advocacy service providing coordination of care for all levels and ages collaborating with the healthcare system inclusive of physicians, hospitals, independent elderly communities, memory care units and the community  are an award winning company with a passion for what we do. Team oriented with professional environment.

Responsibilities include coordinating care, managing medications, physicians appointments, crisis intervention with hospitalizations, community involvement to explain who we are and what we do, must have strong collaborative experience and the ability to challenge the system when they don't do what they should. The role will eventually be certified as a dementia practitioner and crisis interventionist paid for by the company. This is a very independent role for the right person.
