Licensed Practical Nurse

CVS Health is seeking a compassionate Licensed Practical Nurse to deliver high-quality care in our healthcare settings. In this role, you'll provide direct patient care including vital signs, medication administration, injections, wound care, and health education under RN or provider supervision. You'll coordinate with an interdisciplinary team, maintain accurate electronic health records, and support chronic disease management and preventive care. Responsibilities

• Provide direct patient care including vital signs, injections, and wound care
• Administer medications and vaccinations per provider orders and protocols
• Document care accurately in electronic health records (EHR)
• Support chronic disease management and preventive health services
• Assist providers and RNs with exams, procedures, and care plans
• Educate patients on medications, treatments, and healthy lifestyles
• Monitor patient status and promptly report changes to the clinical team
• Follow infection control, safety, and quality standards
• Coordinate referrals, follow-up visits, and care transitions
• Collaborate with an interdisciplinary team to improve patient outcomes
