As a Medical Records Coordinator, you'll play a critical role in ensuring accurate clinical documentation, maintaining compliant medical records, and supporting the reimbursement process that helps our residents receive the care they need. This Medical Records Coordinator position is ideal for someone with skilled nursing experience who thrives on organization, accuracy, collaboration, and making an impact through excellent documentation and record management.

What You'll Do
  • Manage resident medical records from admission through discharge, ensuring documentation is accurate, complete, and compliant with state and federal regulations.
  • Prepare and maintain resident charts, upload clinical documentation, and organize records within Point Click Care  (PCC) and other electronic systems.
  • Support the MDS and reimbursement process by gathering, reviewing, and submitting documentation required for insurance appeals and payer reviews.
  • Conduct chart audits to ensure physician orders, diagnoses, signatures, visits, and other required documentation are completed timely.
  • Coordinate with hospitals, physicians, nursing leadership, and interdisciplinary teams to obtain and maintain required medical documentation.
  • Manage discharged and expired resident records, including chart closure, record retention, and destruction processes.
  • Assist with state surveys, audits, compliance reviews, and requests for records from regulatory agencies and insurance providers.
  • Serve as a resource for medical records procedures, regulatory updates, and documentation best practices while helping train backup staff and supporting continuous process improvement.
Minimum Qualifications
  • High school diploma or equivalent required.
  • Two or more years of experience in healthcare, long-term care, skilled nursing, rehabilitation, or a related healthcare setting.
  • Knowledge of medical records processes, documentation standards, and healthcare confidentiality requirements.
  • Strong organizational skills with exceptional attention to detail and accuracy.
  • Proficiency with computer systems, electronic health records, and Microsoft Office applications.
  • Knowledge of medical terminology and diagnosis coding.
  • Ability to prioritize multiple responsibilities and meet deadlines in a fast-paced environment.
  • Strong verbal and written communication skills.
Preferred Qualifications
  • Accredited Records Technician Program, Health Information Management education, CNA certification, or equivalent experience.
  • Skilled Nursing Facility (SNF) or Long-Term Care (LTC) medical records experience.
  • Experience supporting insurance appeals, MDS documentation, Medicare/Medicaid reimbursement, or case mix processes.
  • Point Click Care  (PCC) experience.
  • Experience in skilled nursing administrative roles such as Medical Records Coordinator, Staffing Coordinator, Scheduler, Central Supply Coordinator, Health Unit Coordinator, or Unit Secretary.
  • Knowledge of long-term care regulations, surveys, audits, and compliance processes.
  • Experience in skilled nursing administrative support roles, including:
    Central Supply Coordinator, Health Unit Coordinator, Unit Secretary, Medical Records Coordinator
  • Knowledge of state and federal long-term care regulations and survey processes.
  • Experience participating in audits, regulatory reviews, and compliance documentation requests.
