The Med Care Manager/QMAP administers, pours, passes, and documents all routinely prescribed medications per physician orders under the direction of the Wellness Director and in accordance with State Medication Administration Regulations. The role involves learning each resident’s medication preferences and ensuring timely and safe medication administration.
Responsibilities- Administer and document regularly prescribed medications per the Medication Administration record, observing the five rights for safe administration.
- Order medications from the pharmacy when needed, ensuring supplies are available and notifying the Wellness Director within 24 hours if not received.
- Document administered medications in the resident’s medical record immediately after administration.
- Observe and report reactions or side effects to the licensed nurse.
- Record vital signs prior to medication administration as ordered.
- Administer PRN medications per physician order.
- Report any medication error to the Wellness Director immediately.
- Assist in developing and reviewing the resident’s care plan with other disciplines.
- Ensure residents’ privacy, respect, and dignity.
- Detect, correct, and report unsafe conditions that could harm a resident.
- Assist with daily care tasks such as toileting, grooming, bathing, feeding, dressing, and moving residents.

- Successful completion of state‑approved Medication Administration training;
First Aid certification as required by the ALF regulations; QMAP in CO. - Minimum high school diploma or equivalent; preferred prior experience in health‑care, assisted living, or memory care, with a record of medication administration.
- Ability to read, write, and communicate in English.
- Knowledge of medication administration procedures and universal precautions.
- Minimum three years’ experience in senior living preferred; passion for working with seniors.
