Post Rehab is seeking a Physical Therapist Assistant (PTA) to join our rehabilitation team. This is an excellent opportunity to work alongside experienced clinicians while helping residents improve mobility, independence, and overall quality of life in a skilled nursing and post-acute care setting.

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist
• Assist residents with therapeutic exercises, gait training, transfer training, balance activities, and mobility programs
• Carry out individualized treatment plans designed to improve functional outcomes
• Monitor resident progress and communicate updates to the supervising Physical Therapist
• Collaborate with nursing and interdisciplinary team members to support coordinated resident care
• Complete accurate and timely clinical documentation in accordance with facility standards

• Collaborative rehabilitation team including PT, OT, SLP, and nursing professionals
