We are seeking a dedicated and compassionate Hospice Nurse Case Manager to join our team in Baker City. In this full-time role, you will provide comprehensive nursing care and manage the overall care plan for patients receiving hospice services. Your expertise will ensure that patients and their families receive the best possible support during a challenging time.
Key Responsibilities:
- Conduct thorough assessments and develop personalized care plans.
- Coordinate patient visits aligned with needs and physician orders.
- Manage timely medication administration and orders.
- Supervise LPNs, LVNs, and Hospice Aides per protocols.
- Collaborate with interdisciplinary team for holistic physical, emotional, and spiritual care.
- Educate families and caregivers with supportive resources.
- Maintain compassionate, respectful approach with empathy and integrity.
Qualifications:
- Associate's degree preferred;
Registered Nurse license required. - At least one year of nursing experience, with prior hospice experience favored.
- Willingness to participate in on-call rotations.
- Possession of reliable transportation and a valid driver's license.
