Equity Staffing is seeking a compassionate and experienced Hospice Registered Nurse (RN) that is focused on providing quality end-of-life care through case management, admissions, and ongoing support for patients and families in a rural community setting.

  Baker County
• Schedule:
  
  Monday-Friday, 8:00 AM-4:30 PM
• Weekly Guarantee: 32 hours
• Mileage Reimbursement:
  Yes, between patient visits
• On-Call Requirement:
  • One weekday night every other week
  • Weekend call rotation shared among four nurses

• Manage hospice patient caseloads and coordinate individualized care plans
• Complete hospice admissions and assessments
• Provide compassionate nursing care and symptom management
• Educate and support patients and families throughout the hospice journey
• Collaborate with interdisciplinary care teams to ensure quality patient outcomes
• Maintain accurate and timely clinical documentation
• Respond to patient needs and changes in condition appropriately

• Active RN license
• Previous hospice nursing experience required
  
  Experience with Point Care documentation system
• Strong clinical assessment and communication skills

• Strong critical thinking and organizational skills
• Ability to work independently in the field
