About the Role Heart 'n Home Hospice is seeking a Hospice Nurse Practitioner to provide palliative care, symptom management, and supportive care for hospice patients. The NP works closely with the Hospice Medical Director, referring physicians, patients, families, and the interdisciplinary hospice team to ensure high-quality end-of-life care.

Key Responsibilities

  • Complete face-to-face hospice visits for qualifying patients
  • Assess patient condition and report findings to the Hospice Medical Director
  • Collaborate with physicians and interdisciplinary hospice team members
  • Develop, implement, and coordinate individualized plans of care
  • Conduct symptom management and supportive care consultations
  • Educate patients and families on self-care and disease management when appropriate
  • Maintain accurate clinical documentation and communicate changes in patient condition
  • Support patients and families through advanced illness and end-of-life care

Required Qualifications

  • Current Nurse Practitioner certification in the state of practice
  • National certification (ANCC, AANP, NBCHPN, or equivalent)
  • Current CPR certification
  • 5+ years of nursing experience
  • Valid driver's license, reliable transportation, and vehicle insurance

Preferred Qualifications

  • Hospice, palliative care, oncology, geriatrics, or ICU experience
  • Experience working within an interdisciplinary care team
  • Palliative Care ARNP experience

Compensation for this specialty generally ranges from $104,500 to $156,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
