Provide intensive, personalized care to medically complex patients. Educate and empower patients to manage their conditions effectively. Build lasting relationships and guide patients toward stable health and self-management. Workload and structure:
See up to 14 patients per day. Partner with experienced nurses and support staff for seamless care delivery. Ideal candidate:
Board-Certified or Board-Eligible in Internal Medicine . Hospitalist experience strongly preferred for managing complex, high-acuity cases. Passionate about chronic disease management and prevention. Strong communicator who values teamwork and long-term patient connections. Committed to proactive, holistic care.
