Essential Duties and Responsibilities
  • Serve as a point of contact with 3rd Party entities and provide technical assistance related to partnership agreement provisions.
  • Support the Manager of Wellness & Prevention Partnerships in negotiating and finalizing partnership agreements with 3rd Party entities.
  • Assist and facilitate in coordinating meetings and communications related to partnership agreement negotiations and implementation, ensuring timely follow-up and documentation of agreements.
  • Works closely with KHS stakeholders to document and communicate policies and procedures related to closed loop referral systems.
  • Coordinates training and education between KHS and 3rd Party entities.
  • Assist with identifying and tracking quality improvement activities and milestones of partnership agreement negotiations and implementation, providing regular progress updates to the Manager of Wellness & Prevention Partnerships.
  • Assist in developing and maintaining comprehensive documentation of Partnership agreements, including terms, conditions, and timelines.
  • Support the evaluation and monitoring of partnership agreement compliance and performance, identifying areas for improvement and strategic adjustments as needed.
  • Perform other job-related duties as assigned.
CORE COMPENTENCIES / KNOWLEDGE & SKILL REQUIREMENTS
  • Understanding of partnership agreement negotiation processes and best practices.
  • Knowledge of wellness and prevention programs and strategies.
  • Strong communication and interpersonal skills, with the ability to effectively engage with diverse stakeholders.
  • Excellent organizational and time management skills, with the ability to manage multiple tasks and priorities simultaneously.
  • Proficiency in Microsoft Office suite and project management tools.
  • Familiarity with healthcare regulations and compliance requirements related to wellness and prevention programs.
  • Demonstrates knowledge on the preparation and planning for coordinated programs with community-based organizations, local government agencies, local education agencies, private agencies and health care providers.
  • Ability to plan and coordinate and track the work of a project team.
  • Communicates effectively with stakeholders of various professional and vocational backgrounds.
Employment Standards Education and experience
  • Possession of a bachelor’s degree in a relevant field such as public health, business administration or a related field from an accredited institution required.
  • Two (2) years of healthcare experience or other job/school related experience required.
  • Attention to detail and accuracy in documentation and record-keeping.
  • Able to use reporting tools, reporting analysis, program development, and Microsoft Office Suite.
