This is a full-time, on-site role for a Home Health Aide located in Santa Rosa, CA. The Home Health Aide will assist patients with their day‑to‑day needs, including personal care, activities of daily living, and ensuring their comfort and dignity are maintained. The role involves direct patient care, home care assistance, and supporting patients with compassion and respect while working as part of a collaborative team committed to excellent hospice services.

• Skills in Personal Care, including bathing, grooming, and hygiene assistance
• Experience with Activities of Daily Living (ADLs) such as mobility support, meal preparation, and household tasks
• Patient Care proficiency and a compassionate approach to providing support
• Strong Communication skills to interact effectively with patients, families, and team members
• Demonstrated expertise in Home Care with knowledge of safety and health practices
• Relevant certifications such as Home Health Aide (HHA) certification
• Ability to work collaboratively in a team environment and adapt to patient-specific needs
