LVN Case Manager

Rosewood, a distinguished award-winning Human Good life plan community in Bakersfield, CA is seeking a Full‑time LVN Case Manager for Health Center (skilled nursing facility). The LVN case manager works with new residents to assess their conditions and requirements with the purpose of developing care plans customized to meet the specific personal needs.

Full‑time, Monday - Friday, 9:00 AM - 5:00 PM $30.00 - $34.00 hour (DOE)

• Work with new residents to assess their conditions and requirements and develop care plans customized to meet their specific personal needs.
• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate their needs, goals and interventions.
• Coordinate implementation of the plan of care and ensure care coordination and case management for each patient; apply specific criteria for admission and re‑certification to establish appropriate levels of care and patient eligibility.
• Assess residents’ needs at move‑in and continually evaluate their condition, promoting the highest functional level of independence possible while complying with state and federal regulations and community safety procedures.
• Develop discharge plans and ensure quality and cost effectiveness.
• Develop assignments in coordination with the care partners and assist in supervising team members accountable for resident care and satisfaction.

• Current CPR certification.
• 2 years case management experience.
• Minimum of 2 to 4 years professional managerial/supervisory experience in a related field.
• Excellent clinical observation and assessment skills.
• Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members.
