We are seeking an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive primary care services to seniors enrolled in a Program of All-Inclusive Care for the Elderly (PACE). Working collaboratively with the Medical Director and interdisciplinary care team, the NP/PA will assess, diagnose, treat, and manage acute and chronic health conditions while coordinating participant-centered care across clinic, home, hospital, nursing facility, and community settings.
This role is focused on promoting independence, improving health outcomes, and enhancing quality of life for older adults.
- Perform comprehensive health assessments, physical exams, and periodic reassessments while developing individualized care plans.
- Diagnose and manage acute and chronic conditions by ordering/interpreting diagnostic tests, prescribing medications, and performing procedures within scope of practice.
- Collaborate with physicians, nurses, specialists, caregivers, and the interdisciplinary team to ensure coordinated, continuous care.
- Provide primary care services in clinic, participant homes, hospitals, nursing facilities, and community settings as needed.
- Monitor participant progress, adjust treatment plans, provide patient/caregiver education, and promote preventive care.
- Maintain accurate and compliant electronic medical records (EMR).
- Participate in interdisciplinary meetings, quality improvement initiatives, and case conferences.
- Ensure compliance with clinical standards, regulatory requirements, and patient safety protocols.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current, unrestricted California NP or PA license.
- DEA registration (or ability to obtain within six months of hire).
- Minimum 5 years of clinical nursing or advanced practice experience preferred.
- At least 1 year of experience caring for frail, elderly, or geriatric populations required.
- Experience in PACE, primary care, geriatrics, outpatient care, home health, or community health preferred.
- Experience working within an interdisciplinary healthcare team preferred.
- Supervisory or leadership experience is a plus.
- Current BLS/CPR certification.
