Service Coordinator

San Gabriel/Pomona Regional Center (SGPRC) is seeking a dedicated Service Coordinator to join our team, dedicated to facilitating and managing services for individuals with developmental disabilities. In this role, you will act as a liaison between our clients, their families, and service providers, ensuring seamless coordination and optimal support. This position requires a compassionate professional capable of assessing client needs, developing individualized service plans, and monitoring outcomes to enhance the quality of life for those we serve.

Responsibilities

• Conduct initial needs assessments for new clients and develop personalized service plans.
• Regularly review and modify service plans in response to changing needs of clients.
• Coordinate with service providers to ensure that clients receive necessary services.
• Assist families and clients in understanding available resources and support services.
• Monitor and evaluate the effectiveness of services provided to clients and make adjustments as necessary.
• Maintain accurate and detailed client records in compliance with regulatory requirements.
• Provide crisis intervention as needed and coordinate with other professionals to provide comprehensive support.
