Extended Services Coordinator

THIS IS A NON-CIVIL SERVICE POSITION

Compensation

Salary Range: $35,120.00 – $43,901.00 Annually.

Hiring Salary Range: $35,120.00 - $39,510.50 Annually.

Job Summary

The Baltimore City Health Department (BCHD) seeks an Extended Services Coordinator. Founded in 1793, BCHD is the oldest health department in the country, with over 800 employees and an annual budget of $208 million, that aims to protect health, eliminate disparities, and ensure the well‑being of every resident of Baltimore through education, advocacy, and direct service delivery. The Extended Services Coordinator will advise clients on available City, State and private services and programs;

provide clients with handouts, brochures and related documents for their information and use.

Responsibilities
  • Develop collaborative relationships with families while maintaining professional boundaries.
  • Determine with families their concerns, priorities and needs of the child and family.
  • Jointly develop and revise Individualized Family Service (IFSP) plans.
  • Effectively monitor and adhere to the required timelines for Part C and Part B and maintain all required paperwork and documentation in the Maryland Online IFSP.
  • Maintain accurate, up‑to‑date and detailed client case records and files; ensure the confidentiality of client case records and files.
  • Write reports, correspondence and evaluations concerning the status of clients, problem resolution and long‑term plans and strategies.
  • Enter client data into a database record system.
  • Provide clients with handouts, brochures, and related documents.
Minimum Qualifications
  • Education:

    Graduation from an accredited high school or possession of a GED certificate.
  • Experience:

    Six months of experience performing administrative duties.
  • Licenses, Registrations, and Certificates: A valid Maryland Class C Non‑commercial driver’s license or an equivalent driver’s license and eligibility to obtain a Baltimore City driver’s permit. Provisional driver’s licenses are not acceptable.
  • Applicants must upload a copy of their valid driver’s license with the application at the time of applying.
Knowledge, Skills, and Abilities
  • Maintains accurate, up‑to‑date and detailed client case records and files; ensures the confidentiality of client case records and files.
  • Writes reports, correspondence and evaluations concerning the status of clients, problem resolution and long‑term plans and strategies.
  • Enter client data into a database record system.
  • Ability to communicate effectively, both orally and in writing.
Additional Information
  • Background Check:
    Eligible candidates under final consideration for appointment to positions identified as positions of trust will be required to complete authorization for a Criminal Background Check and/or Fingerprint screening, and must be successfully completed.
  • Probation:
    All persons, including current City employees, selected for this position must complete a 6‑MONTH mandatory probation.
  • NOTE:

    GRANT POSITIONS ARE NOT ELIGIBLE FOR RETIREMENT BENEFITS.
