PCMH model clinic in Baltimore is seeking a dynamic, mission-driven Psychiatrist to put the principles of team-based care and population health into action for our medically complex & socially vulnerable members. This position is mostly tele-psychiatry with required visits quarterly at clinics in Baltimore, Greenbelt MD and Philadelphia. It's not necessary to live near these areas but a MD or PA license is required.

(Clinic will eventually help acquire both state licenses.)Unfettered from the traditional fee-for-service model, through unique provider-payor relationships, this healthcare organization is designed to care for societys sickest patients. Prior to coming to this clinic, their patients are typically ill-served by the uncoordinated traditional health care system, bouncing between emergency rooms, hospitals, and specialists several times a year. The role of their PCPs is break the status quo and transform our members health & lives.

In order to do so, Their Physicians lead an integrated care team of nurse care managers, social worker, behavioral health specialist, and community outreach workers to deliver complex primary care that addresses the patients health and life issues. They offer complex, coordinated medical care through our medical teams with our onsite infusion center, full pharmacy, and lab & imaging suites.

They also treat the social determinants of health, addressing housing instability, food insecurity, or simply loneliness through our integrated care team.

Roles & Responsibilities Aid with conducting relevant administrative duties as required to include:

- Collaborate with National Director in monitoring and evaluating productivity and impact of services over time - Collaborate with the National Director on relevant policy development - Engage in quality assurance activities (i.e. chart reviews, audits, technical assistance, case consultation etc.) Provide Clinical Coordination and Assistance with Medication Management for Members with acute and chronic Behavioral Health needs to include:

- Co-directing treatment with primary medical provider through application of a collaborative care model - Provision of education and ongoing technical-assistance to on and offsite providers related to medical management for BH population - Provide direct patient care to members requiring evaluation and access to psychiatry direct care to include provision of diagnosis; recommendations for treatment; evaluation of treatment plan; evaluation of treatment adherence etc.

- Available for peer to peer consultations as needed for successful management of member care Support with Transitional Care Management & Emergency Department diversion & Crisis Intervention
- Provide interdisciplinary collaboration regarding the successful management of high risk, high utilizing members with co-occurring behavioral health concerns - Perform clinical review as needed for high risk members - Advice and assist with prescribing behavioral health medications - Support with medication reconciliation especially post ED encounter and post hospitalization encounter - Available for peer to peer consultations as needed for successful management of member care Serve as Pain Management Subject Matter Expert (SME) for Absolute

- Creation and implementation of clinical protocols for tapering - Creation and implementation of clinical protocols for Medication-Assisted Treatment - Co-directing treatment with primary medical provider - Provision of education and ongoing technical-assistance to on and offsite providers related to Pain Management Co-Develop local Addiction Treatment with emphasis on Medication-Assisted Treatment
