Job Overview

We are seeking a Hospice Chaplain to join our amazing Hospice Team! The Chaplain provides spiritual support to the Professional Healthcare Resources Hospice Program. Spiritual needs are assessed and direct ministry to patients and families is provided through spiritual guidance and counseling. Duties require an informed respect for the belief system of others.

Responsibilities
  • Spiritual and pastoral counseling services are provided in accordance with the plan of care and include:
    • Spiritual and pastoral assessment is completed as soon as possible after admission to hospice.
    • Direct ministry, counseling and guidance to hospice patients and families in accordance with ongoing wishes and needs after admission to hospice and at the time of death when possible.
    • Develop an individualized spiritual and pastoral plan of care.
    • Offer patients and families of different philosophies and religious beliefs opportunities to discuss and share thoughts, feelings, beliefs and values.
    • Document spiritual assessments, care plans for future patient intervention and ongoing visits.
    • Work with other professionals in resolving spiritual, pastoral, and ethical issues.
    • Work with the interdisciplinary hospice team to evaluate patient and family response to care.
    • Bereavement visits to the patient’s family prior to death and availability to the family during the bereavement period if needed.
    • Meet with individual hospice staff regarding personal spiritual and pastoral issues that may affect their ability to function effectively.
  • Collaborate with hospice staff to integrate spiritual care into the overall plan of care for each patient.
  • Provide support to hospice staff in addressing ethical and spiritual issues that arise during care.
Requirements
  • Must have a degree or certification that supports the ability to perform in this role.
  • Two (2) years of pastoral counseling experience.
  • At least one-year of counseling terminally ill patients and their families in a hospice or similar setting (preferred).
