Job SummaryThe Safe Sleep coordinator provides safe sleep and family planning education, assessment, referral, and provision of a portable crib to qualifying Baltimore City families. The coordinator also provides prenatal and post‑partum health education to women referred to the Safe Sleep program. Responsibilities include client assessments, performing health education, completing home delivery and set‑up of portable cribs to eligible referred clients, attending community outreach events to provide Safe Sleep education to clients, families, and partner organizations, and linking families to family planning services and other resources such as WIC, Infants and Toddlers, and Home Visiting.
Dutiesand Responsibilities
- Case Management: receive, track, review, and document case services for Safe Sleep clients; outreach all postpartum clients within 48 hours of case receipt; outreach all pregnant clients within 5 business days of case receipt; ensure 100% on‑time closure for Safe Sleep outreach; provide education on Safe Sleep practices and available resources; provide linkage to Family Planning Services.
- Documentation: document initial outreach within 48 hours; document crib delivery and upload receipt within 1 business day; maintain database of referrals and outreach attempts; maintain inventory of cribs and materials; complete monthly reports of crib distribution; provide monthly updates to the MCH Community Liaison/Supervisor; assist in record preparation for city‑wide meetings.

Education:
deliver cribs to clients’ homes with set‑up demonstration and re‑demonstration; participate in community outreach events to share educational materials. - Human Resources: consistent and timely communication with supervisor; submit timesheets accurately; complete required training courses.
- Additional duties: additional duties as assigned.
- High School Diploma or GED with 3 years of human services experience required;
Bachelor of Human Services or related field with 1 year of human services experience preferred. - Community Health Worker Certification in Maryland preferred.
- Spanish language fluency preferred.
- Demonstrated knowledge of Community Outreach, Health Education, and familiarity with the prenatal/postpartum newborn population.
- Comfortable with making home visits in Baltimore City; reliable transportation to transport portable cribs.
- Knowledge in Safe Sleep, Community Outreach, Maternal and Child Health, or Medicaid System Navigation.
- Experience working with pregnant and postpartum individuals.
- Strong computer literacy: MS Word, Excel, Outlook, Powerpoint, SharePoint (365).
- Proficiency with Electronic Medical Records (EMR) systems.
- Organizational skills, ability to meet demanding workload, sensitivity to cultural and socioeconomic characteristics.
- Excellent oral and written communication; proven ability to work efficiently and meet deadlines.
- Strong knowledge of community resources.
