Pediatric Inpatient Oncology - Clinical Pharmacy Specialist

Overview

The Clinical Pharmacy Specialist – Pediatric Inpatient Oncology provides comprehensive pharmaceutical care to pediatric oncology patients in the inpatient setting as a member of the interdisciplinary care team.

• Clinical review and verification of chemotherapy and supportive care regimens.
• Participation in daily rounds and medication reconciliation at transitions of care.
• Therapeutic drug monitoring and identification and management of medication-related problems.
• Monitoring for treatment-related toxicities and supporting safe use of high-risk medications.
• Oncology formulary management: evaluation of new medications, maintenance of protocols and order sets, utilization review, and participation in Pharmacy & Therapeutics initiatives.
• Coordination of complex treatment plans, involvement in quality improvement and medication safety initiatives, and education of pharmacy students and residents.

• Graduation from an accredited College of Pharmacy resulting in a Pharm.
  
  D. degree, or B.S. with 1 year of experience.
• Completion of an ASHP-accredited residency program or equivalent experience; specialty residency in Pediatrics is preferred.
• Current licensure as a pharmacist in the State of Maryland.
• Board Certification through the Board of Pharmacy Specialties or a practitioner-specific certification deemed appropriate by the Division Director within 2 years of start date.
• BLS certification within 1 year of hire.
