The Department of Pediatrics is seeking a full time compassionate, bilingual (Spanish/English)
Clinical Program Coordinator to support clinical care coordination, patient navigation, and program development for immigrant survivors of human trafficking, torture, and other forms of trauma in the Hospitalist division for the HEAL program. The successful candidate will play a key role in linking vulnerable populations—particularly Spanish‑speaking youth and families—with medical, behavioral health, and community‑based services. Embedded within both Johns Hopkins and Asylee Women Enterprise (AWE), the Clinical Program Coordinator serves as a bridge across clinical and community spaces, offering culturally humble, trauma‑informed support to ensure clients are able to access and engage with care.

The coordinator will also support the implementation of grant‑funded initiatives, including the Steps to Success and Survivors of Torture programs.

Specific Duties & Responsibilities Client Navigation & Clinical Coordination
  • Provide trauma‑informed, culturally sensitive health navigation for clients, including survivors of human trafficking and torture.
  • Conduct screenings for behavioral health and substance use concerns using validated tools; deliver brief interventions for mild behavioral health concerns under clinical guidance.
  • Assist clients in understanding and navigating the healthcare system, including insurance enrollment, referrals, and follow‑up appointments.
  • Coordinate directly with therapists, physicians, psychiatrists, and case managers to support ongoing care plans.
  • Provide ad hoc Spanish‑language interpretation to support client communication and clinical encounters.
  • Support the implementation and facilitation of Spanish‑language peer support and psychoeducation groups for youth and caregivers.
Program Implementation & Operational Support
  • Maintain accurate and timely documentation in HIPAA‑compliant systems; ensure adherence to grant and institutional reporting requirements.
  • Collaborate with HEAL leadership and partners to implement and refine the Behavioral Health Integration infrastructure within the HEAL Primary Care program.
  • Contribute to the design, curriculum development, and facilitation of psycho educational groups for immigrant youth and families.
  • Maintain a robust referral network to primary care, specialty services, mental health care, and supportive community‑based resources.
  • Assist in managing the pharmacy delivery program for patients, coordinating medication orders and tracking distribution.
Community Engagement & Outreach
  • Build and maintain trusting relationships with clients to better understand their barriers to care and offer holistic, sustainable solutions.
  • Serve as a liaison between HEAL clinicians and community partners at AWE and other organizations, helping to coordinate care across systems.
  • Participate in community outreach and engagement activities; serve as a resource to community leaders and partner organizations.
  • Attend and contribute to case conferences, staff meetings, multidisciplinary care discussions, and program planning sessions.
Quality Improvement & Evaluation
  • Track and monitor programmatic outcomes related to clinical access, client satisfaction, and care coordination.
  • Support grant compliance through detailed case tracking and data collection aligned with funder requirements.
  • Contribute to continuous quality improvement efforts by integrating client feedback and participating in HEAL’s M&E initiatives.
