We help individuals make informed decisions about their reproductive health, family planning options, and sexuality.

Under the general direction of the Director of Patient Access, the Patient Access Coordinator helps individual patients navigate barriers to accessing Family Planning, Gender‑Affirming Care and Abortion services by facilitating delivery of cost estimates, managing programs, and connecting them to appropriate resources & supports. This role supports PPM's commitment to providing needed resources for patients seeking care in Maryland by facilitating coordination of care and providing patient‑centered solutions.

This position will both expand navigation support available to patients at PPM and help to build sustainable systems that will bolster access for the future.

Schedule: M‑F (8am‑5pm)

Location: Primary location Baltimore, MD; will be required to work at other Health Centers in PPM network as needed.

Essential Responsibilities
  • Ensure patient is receiving services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences
  • Including, but not limited to scheduling patients for appointments, completing financial screenings, and assisting with funding support
  • Conduct insurance error mitigation tactics to create a positive experience for our patients
  • Manage specialized programs including Abortion Funding Support, Gender‑Affirming Care and Presumptive Eligibility programs and others
  • Conduct periodic training sessions with clinical staff to help promote programs
  • Work with the patient to identify logistical needs to access care and link the patient to appropriate resources as needed; including but not limited to transportation, lodging, and medical services, and linking the patient to financial resources
  • Support the coordination of care by collaborating with affiliates outside of PPM to prevent unnecessary or redundant care; and to help reduce the cost of care when appropriate
  • Work with the patient to identify other support services as needed and provide appropriate linkages
  • Identify and arrange interpretive language services when needed
  • Take the initiative to proactively resolve barriers, support and/or escalated tough patient cases via communication to management, providers, and patients
Program Development and Community Partnership
  • Collaborate and partner with internal and external stakeholders to facilitate and create clear workflows for patients with complex needs seeking care end meetings with external stakeholders and develop strong relationships to ease the coordination of care for patients
  • Meet key program metrics, collaborate with manager in collecting, tracking, and reporting on required program data
  • Collect data and document trends at the Center level to support informing and moving forward PPM’s strategic Access expansion initiatives
  • Facilitate patient care with external providers, in collaboration with outside navigation staff, to maintain continuity of care for patients traveling from out‑of‑state to Maryland for abortion care. Includes ensuring that appropriate appointment-making, transmission of medical records, and other navigation logistics are tracked and resolved for out‑of‑state patients as necessary
  • Work in partnerships with other departments, including External Relations & Development team, by being a subject matter expert on the support work done by the PAC team
Experience and Skills
  • High school diploma or GED required
  • Minimum of 2 years of experience providing Patient Access support work in a Healthcare field required
  • Bilingual in Spanish and English highly preferred
  • Program Management experience preferred
