Primary Responsibility and Duties
- Conducts assessments of client needs, support networks, isolation risk factors, financial status, living conditions, and functional ability.
- Mitigate concerns identified during the assessment process and those monitored during delivery.
- Provides ongoing service linkages within MOWCM and the community, and associated intervention for high-risk clients; monitors progress/status through face‑to‑face visits, and check‑in telephone calls to ensure high quality, ongoing care that meets the clients' specific needs, and monitors' progress.
- Works with families and partners as appropriate to communicate concerns, offer support, and coordinate care to improve nutrition, independent living, and health.
- Provides targeted information to clients regarding appropriate community resources, facilitates process of application for services and offers education in the areas of nutrition, health, and home safety.
- Promptly and consistently documents all service interventions using the client data base, providing progress note summaries, and re‑assessments as necessary.
- Provide training, assistance, mentoring, and oversight to other staff and interns who assist in identifying and serving high‑risk clients.
- Network with other agencies to make them aware of Meals on Wheels of Central Maryland, Services, learn of their services, and create two‑way referral networks.
- Stay abreast of community resources and best practices in senior nutrition and other services and maintain an up‑to‑date resources lists/materials manuals for assigned regions.
- Address all safety concerns and other emergent needs expediently, working with Adult Protective Services, Police, Medical Personnel, and others as needed.
- Protects the confidentiality and integrity of Protected Health Information (PHI) according to Meals on Wheels' relevant policies and procedures.
- Views, alters, retrieves, and stores PHI using company‑defined, secure methods consistent with the tasks required of them.
- Bachelor's degree in social work or a closely related human services field
- Two years related work experience, supervisory experience. Master's degree accepted in lieu of one year's experience.
- Current driver's license, dependable licensed and insured vehicle
- Able to pass criminal background check.
To be considered for employmnet with Meals on Wheels of Central MD, you must pass both a background check and complete a drug screen.
