Physical Therapist
Role Overview:
Provide therapy services through home visits, managing an average of 5-6 patients daily. Participate in periodic meetings, document care accurately, and meet productivity targets of approximately 30 points weekly, equating to around 40 hours of work. Flexibility to travel up to 75 miles and potentially float to nearby locations is required. Proficiency in EMR systems and documentation standards is essential.
- Conduct home health therapy visits for adult patients.
- Maintain accurate documentation and meet productivity goals.
- Travel as needed within designated radius, with mileage reimbursement.
- Participate in meetings and adhere to agency protocols.
- Minimum 1 year of recent adult home health experience.
- Valid license and current CPR, BLS certifications.
- Reliable transportation and liability insurance.
- Proficiency in EMR systems.
