Job Summary
The Entitlements Case Manager will provide case‑management services to clients in community‑based group homes and other settings across three organizations:
Lighthouse Behavioral Health Center, Community Wellness Outpatient Mental Health Center, and Community Wellness Psychiatric Rehabilitation Program. The position is available as a part‑time W‑2 employee or a 1099 independent contractor.
- Facilitate individual and group case‑management sessions, intake orientation, and psycho‑education groups.
- Conduct assessments, complete 6‑month reassessments, and maintain monthly plan reviews.
- Assist clients in applying for and maintaining eligibility for Medicaid, SNAP, SSI/SSDI, TCA, housing, and other benefits.
- Coordinate with internal staff and external providers (e.g., housing agencies, probation, substance‑use programs) to address social determinants of health.
- Maintain accurate documentation in the electronic health record (ICANotes) and comply with CARF, COMAR, Medicaid, and other regulatory requirements.
- Participate in audits, quality‑improvement activities, and competency trainings; support new‑hire orientation and continuing education efforts.
- Respond to crisis or safety situations and provide appropriate referrals as needed.
- Complete required documentation within specified time frames (e.g., 24–48 hours for certain services).
- Maintain confidentiality and uphold HIPAA standards; use encrypted devices and secure data storage.
- Bachelor’s degree in psychology, social work, counseling, nursing, or a related field (Master’s preferred).
- Valid Maryland license or certification in one or more of the following: LCADC, LGADC, CAC‑AD, CSCAD, ADT, LCPC, LGPC, LCSW‑C, LCSW, LMSW, LBSW, LCMFT, LGMFT, psychologist, registered psychology associate, CRNP‑PMH, APRN‑PMH, CNS‑PMH, or eligible student intern.
- Minimum 2 years of experience in behavioral health, community‑based case management, health‑home coordination, or psychiatric rehabilitation.
- Proficiency with ICANotes, CRISP, or other EHR/HIE platforms; strong documentation and organizational skills.
- Trauma‑informed, culturally responsive practice with knowledge of Medicaid, COMAR, and CARF standards.
- Excellent communication, time‑management, and collaboration skills.
- Ability to work independently and as part of a multidisciplinary team.

Part‑time, 20 hours per week (approximately 50% of total weekly hours) across the three organizations. Work is scheduled Monday–Friday, 8:00 AM–5:00 PM with up to 10% travel. Compensation ranges from $30,000 to $100,000 per year for W‑2 employees or fee‑for‑service rates for independent contractors.
Work Environment- Community‑based or hybrid setting; in‑office, community‑home, or telehealth as appropriate.
- May be exposed to emotionally charged or crisis situations; requires regular walking, standing, light lifting.
- Must drive when required; may travel locally to provider offices or community agencies.
