Main Purpose of Job
The main purpose of this full-time contractual Developmental Disabilities (DD) Targeted Case Management (TCM) Coordinator of Community Services (CCS) position will be to provide a fixed point of entry into the service delivery system for individuals with developmental disabilities. This position will also assist the individual in the preparation of the application and gathering of necessary documentation for DDA to make an eligibility determination.
Further, the CCS is the responsible party for facilitating the development of the Person-Centered Plan (PCP) and monitoring the PCP and its ser5vices. When required, additional assessments are completed as determined by the client's individual situation.

- A Bachelor's degree from an accredited college or university in a Human Services field; OR
- An Associates degree with two (2) years experience in a Human Services field; OR
- Seven (7) years experience in a Human Services field.
Education :
Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience :
One year of administrative staff or professional work in a health care setting.
- Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
- Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.
