Therapist Social Worker

Job Summary:

Provide specialized social work services within the Emergency Department at Sinai Hospital. This role involves assessing patient needs, developing care plans, and coordinating discharge resources to support optimal patient outcomes.

Responsibilities:

• Conduct initial patient evaluations and assess responses to treatment.
• Develop individualized plans of care in collaboration with multidisciplinary teams.
• Coordinate with healthcare providers, home care services, and community resources.
• Facilitate care conferences and participate in multidisciplinary team meetings.
• Manage referrals to additional disciplines, rehabilitation, and long-term care facilities.
• Ensure compliance with advance directive policies and documentation standards.

Qualifications:

• Hold an MSW degree and Maryland LMSW or LCSW licensure.
• Possess at least 3 years of acute care social work experience, preferably in Emergency Department settings.
• Experience with short-term inpatient medical social work and discharge planning is required.
• Demonstrate proficiency in collaborative care, care coordination, and multidisciplinary communication.
• Familiarity with Cerner and AllScripts charting systems.
• Ability to float within scope and adapt to various clinical units as needed.
