LCSW-C (Licensed Certified Social Worker-Clinical) for our Skilled Nursing Facility (SNF)

Compensation: $80,000 - $95,000 Annual Salary

We are looking for a dedicated LCSW-C for our Skilled Nursing Facility in Baltimore, MD. This is a full-time, on-site position. The LCSW-C carries the clinical social work function for the building, covering psychosocial assessment, care planning, discharge planning, and direct support for residents and families, on a caseload that includes substance use and behavioral health needs.

Qualifications of the LCSW-C:
  • Active Maryland LCSW-C license, required. LMSW and LGSW do not meet the requirement for this position.
  • Comfort working with residents who have substance use and behavioral health needs
  • Knowledge of psychosocial assessment, care planning and discharge planning
  • Strong documentation and communication skills
  • Skilled nursing or long term care experience preferred, not required
Responsibilities of the LCSW-C:
  • Complete psychosocial assessments and the social services sections of the MDS
  • Build and update care plans with the interdisciplinary team
  • Coordinate discharge planning and community resources
  • Support residents and families through admission, stay and transition
  • Maintain documentation and regulatory compliance

Compensation: $80,000-$95,000 per year
