Connections That Matter, Inc. is seeking a Bilingual (Spanish-speaking) Direct Care Worker (DCW) to provide community-based psychiatric rehabilitation services through our PRP program.
This role focuses on helping children, adolescents, and adults build essential life skills, improve daily functioning, and successfully integrate into their communities.
Services are delivered in home, school, office, and community settings.
Key Responsibilities- Provide direct support services aligned with the Individual Rehabilitation Plan (IRP)
- Assist clients with developing and improving:
- Daily living skills (ADLs)
- Social and interpersonal skills
- Community integration and resource utilization
- Collaborate with parents, caregivers, therapists, and treatment team members
- Support clients in achieving individualized rehabilitation goals
- Maintain accurate and timely documentation in the Electronic Health Record (EHR) system
- Participate in supervision and monthly trainings
- Maintain professionalism, confidentiality, and compliance with COMAR and agency standards
- High School Diploma or GED Required
- Associate’s or Bachelor’s Degree in a related field Preferred

- Minimum of 1 year experience in mental health or human services
- Strong communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
- Bilingual (English/Spanish) –
REQUIRED
- Valid driver’s license
- Reliable transportation
- Current vehicle registration, inspection, and insurance
- Ability to pass a criminal background check
- Community-based role (home, school, office, and community settings)
