Job Responsibilities
- Coordinate and deliver prevention and wellness education activities to Maryland Medicaid recipients
- Conduct outreach to Medicaid recipients and network providers to promote preventive services
- Research and collaborate with internal, local, state, and federal entities to create an ongoing calendar of prevention and wellness events
- Target Medicaid recipients without recent healthcare practitioner visits
- Identify barriers to healthcare access and refer recipients to Care Management for appropriate support
- Coordinate appointment scheduling and transportation-service referrals
- Deliver health education through virtual, telephonic, and onsite presentations, informational tables, health fairs, community baby showers, and other venues
- Support quality initiatives and NCQA, EQRO, HEDIS, regulatory, contractual, and accreditation requirements
- Maintain and update educational resource libraries
- Develop materials and tools for sponsored programs, events, and Community Advisory Board meetings
- Track and report outreach and educational activities
- Monitor educational presentation outcomes through monthly and ad hoc surveys
- Assist with quarterly member and provider newsletters, including writing and reviewing articles
- Background in managed care, case management, or population health
- Effective communication, public speaking, and influencing skills
- Ability to successfully utilize Microsoft Office suite
- Strong problem-solving, organizational, and time management skills
- Own reliable transportation with appropriate insurance
- Willingness to travel statewide throughout Maryland
- Valid driver's license
- Minimum one year of recent experience with health education, outreach, or working with local health departments, agencies, and/or organizations in underserved communities
- Associate or bachelor’s degree in a health-related field preferred
- Experience with public speaking, creative writing, and graphic arts preferred
Demonstrates expertise in health education and outreach, with a strong focus on coordinating wellness activities and engaging underserved communities. Proficient in managing communication and collaboration with various stakeholders to promote preventive services and meet regulatory requirements.
