Facilitator/Counselor, Bartlett Substance Recovery Service

We are seeking a Facilitator/Counselor for the Bartlett Substance Recovery Service within the East Baltimore Bartlett Infectious Diseases Clinic's RESTORE Recovery Support Service. RESTORE is a multidisciplinary team that supports the clinic in delivering addiction treatment to Bartlett patients through evidence-based medications, harm reduction strategies, individual and group counseling, and care approaches that address the social determinants that affect health and well-being.

Under the direct supervision of the Program Manager, the CAC-AD will ensure fidelity to established protocols and provide evidence-based care and support that promotes participation in addiction and infectious disease treatment for a highly vulnerable patient population. The CAC-AD will be responsible for the day-to-day operations of RESTORE, maintain a visible and intentionally destigmatizing presence in the clinic, and treat all patients and colleagues with professionalism, respect, and compassion.

In addition to maintaining professional certification, the individual in this role will be expected to engage in ongoing learning related to infectious diseases and addiction care, as directed by the Program Manager.

Specific Duties & Responsibilities

Program Facilitation (40%)

  • Maintain a professional presence and ensure the ongoing availability of RESTORE services in Bartlett.
  • Oversee the day-to-day administration and implementation of the RESTORE program in accordance with established protocols.
  • Serve as a liaison between the RESTORE treatment team, patients, and Bartlett clinic RNs, APPs, MDs, and staff to support accurate, timely, and bidirectional communication regarding care plans.
  • Assume primary responsibility for the patient tracker/registry to ensure all patients are monitored appropriately, no one is overlooked, and all documentation and assigned tasks are completed in a timely manner.
  • Prepare for and facilitate weekly interdisciplinary treatment team meetings and communicate treatment plans to relevant care teams.
  • Verify the completeness and accuracy of all data collection, documentation, and tracking related to RESTORE team activities.
  • Maintain working knowledge of clinic resources and actively pursue collaborative opportunities with nursing, social work, pharmacy, psychiatry, and other relevant services.

Direct Provision of Recovery Support Services as a CAC-AD (60%)

  • Actively engage individuals referred to RESTORE and support retention in addiction and infectious disease care that is responsive to their life circumstances and stage of change.
  • Serve, alongside the patient navigator, as a primary point of contact for the RESTORE program, responding promptly to calls and messages.
  • Collaborate closely with the interdisciplinary RESTORE treatment team and implement team recommendations.
  • Provide individual addiction counseling consistent with treatment plans, evidence-based practices, risk mitigation principles, and a strong respect for patient autonomy.
  • Facilitate weekly educational support groups for Bartlett patients who use drugs, alcohol, and/or nicotine, in consultation with the program manager and collaborating psychiatry team members.
  • Support patient adherence to scheduled appointments and medications and reinforce instructions provided by medical staff.
  • Facilitate warm handoffs to higher levels of care and support re-engagement in Bartlett-based services when appropriate. This may include accompanying patients to community-based services such as skilled nursing facilities, residential recovery programs, alcohol detoxification programs, methadone treatment program intakes, hospitals, and emergency departments.
