What You Will Do:
  • Follow individualized plans of care (ISP) for each assigned resident
  • Observe residents for changes in condition and report concerns promptly to the nursing supervisor
  • Document daily care and any notable changes throughout the shift, especially during overnight rounds
  • Check and record vital signs, weights, and perform other duties as assigned in accordance with state regulations and community policies
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, toileting, and oral care
  • Provide restorative nursing support, including positioning, ADL training, and passive range of motion
  • Offer cues, redirection, and emotional support to residents as needed
  • Encourage and assist residents in participating in activities and programs
  • Assist with transfers and ambulation using proper techniques and equipment
  • Respond promptly to call lights and urgent needs
  • Provide transportation within the community and occasionally accompany residents to outside appointments
  • Support activities and other departments as needed
  • Perform other duties as assigned
Why You Are Qualified:
  • High school diploma or equivalent (GED) required
  • Completion of a State-approved Nursing Assistant Training Program (if required)
  • CNA certification preferred
  • CPR and First Aid certification (if required)
  • Ability to perform physical tasks including standing, walking, bending, lifting, and assisting residents throughout the shift
  • Must be able to lift up to 50 pounds and assist with resident transfers using proper equipment
