Inpatient Psych Case Manager PRN LMSW or LCSW

Sinai Hospital's Behavioral Health Case Management team delivers discharge planning, transitional care coordination, and complex care management, staffed by licensed social workers focused on behavioral health patients. The team works closely with physicians and the multidisciplinary care team to ensure every transition of care is clinically safe and effective.

The Inpatient Psych Social Worker at Sinai Hospital acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in the addressing identified needs at any point of the continuum.,

pre-admission to post-discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization. i.e., length of stay and level of care.

This is a PRN position that will be scheduled based on the needs of the dept. including weekend coverage.

• 3-5 years of Inpatient Psych Social Worker experience

• Master of Social Work

• Current Maryland LMSW or LCSW

Pay Range: $43.00 to $43.00
