As a valued member of our team, we're seeking a skilled and dedicated MDS Coordinator to join our prestigious healthcare facility. Responsibilities:- Complete and coordinate all MDS assessments (admission, quarterly, annual, significant change) in compliance with CMS guidelines
- Ensure accurate and timely submission of MDS assessments
- Participate in interdisciplinary team meetings related to resident assessments
- Care Planning
:- Collaborate with nursing, therapy, and other departments to develop individualized care plans
- Ensure care plans reflect residents' current needs, goals, and preferences
- Update care plans based on assessment findings and changes in condition
- Regulatory Compliance
:- Maintain compliance with all federal, state, and facility regulations related to MDS and care planning
- Prepare for surveys and audits by ensuring documentation is complete and accurate
- Stay updated on CMS/RUG/PDPM guidelines and changes
- Clinical Documentation
:- Review medical records to ensure supporting documentation aligns with MDS coding
- Educate nursing staff on proper documentation practices
- Identify and correct discrepancies in clinical documentation
- Reimbursement & Quality Measures
:- Monitor case mix index (CMI) and reimbursement trends
- Assist in maximizing reimbursement through accurate coding
- Track and report quality measures and outcomes
- Collaboration & Communication
:- Serve as a liaison between nursing, therapy, and billing departments
- Communicate effectively with physicians and other healthcare professionals
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
- Minimum 2 years of experience in a healthcare setting
- Strong understanding of MDS processes and regulations
- Excellent communication and interpersonal skills
- Ability to work accurately and efficiently in a fast-paced environment
- Familiarity with electronic medical records and coding systems
- Competitive salary $90K - $110K
